Little Life Warrior Society (LLWS) should ensure that we are serving for the patient/ survivor who diagnosed with cancer, bone marrow transplant or severe blood diseases at the age of 18 years old or under, we shall verify the providing data with medical staff, in order to clarify that who are entitled to receive appropriate service from LLWS. We undertake to comply with the requirements of the Personal Data (Privacy) Ordinance to ensure that personal data kept are accurate and securely kept. To ensure you are well informed of the personal data as collected, please read through the following updates to personal information collection.

Updates to Personal Information Collection

Little Life Warrior Society will use personal data collected from data subject for the purposes for which it was collected. We shall verify the providing data with medical staff if needed.
To provide personal data to the Society is on voluntary basis. However, if you do not provide sufficient personal data, we may not be able to process your application or provide service to you.
The Society may use your personal data in future (including but not limited to name, telephone number, fax number, email, mailing addresses, organization/company/school, diagnosis) for the purposes of providing you with information of the Society, handling application, issuing receipt, research, fundraising appeal, feedback collecting, activities invitation/relates promotion as well as the Society feedback collection purposes.
The Society will keep your personal data (including written and electronic documentaries) will be stored in the system at the Society. Only those who need to know can access the information and retain and remove the personal data kept in accordance with the regulations.

If LLWS does not receive your objection for utilizing your personal data for the above purposes, LLWS shall assume that you have no objection for utilizing your personal data for the above purposes until LLWS receives your further notification.
